The Application Procedure

To apply to Compos Mentis please download the Application and Referral Forms below. Please complete all information on the Application Form and ask the person who prescribes your medication and your psychotherapist, if you have one, to complete the appropriate Referral Form and return it to Compos Mentis. A checklist is included to assist you in making sure your application is complete.

When we have received all the application materials, including referrals from your treatment providers, we will contact you and if we think that you might be able to benefit from our program we will invite you to an Admissions Interview. Preference will be given to those applicants who plan to stay for the entire six months of the Summer Program. To be considered, we require a minimum commitment of one month.

Fees

Our five-day-a-week program costs $600 a month, with payment due one month in advance. We have a limited number of partial scholarships that we provide in cases of documented need. If you would like to request a partial scholarship, please address this in your Admissions Interview.
